Evaluation Process

Prior to the Evaluation
Once an evaluation has been scheduled, a parent or adult client is asked to return the Consent for Services Form (Minor or Adult), and then the parent and/or teachers will be sent rating scales. Along with a completed Family Information Packet, school records are requested. If desired by the client, an Authorization for Release of Information may be signed, and Dr. Eades will consult with other professionals (e.g., teachers, psychiatrist, tutor, etc.). The Family Information Packet (Minor or Adult), school records, and Authorization for Release of Information are requested to be returned to Dr. Eades at least one week prior to the testing date. A few days before the evaluation, an interview is conducted either over the phone or in person, based on convenience for the parent. (An interview with the adult client is conducted during the evaluation.) If the parent feels it would be beneficial, a classroom observation may be scheduled prior to the evaluation. Classroom observations are typically only conducted with elementary age children.

The Evaluation
The evaluation with the child or adult client is typically five to six hours, with short breaks interspersed. Depending on the age/needs of the individual, testing is conducted in either one day or over two days. The global pandemic brought many lessons, and now some services are available using teleconferencing.
For younger children, a two-day evaluation (each session lasting two to three hours each) is recommended.
For older children and adults clients who are participating in a one-day evaluation, they are given the choice to have lunch with Dr. Eades, or they may leave for a one-hour lunch break. If they are having lunch with Dr. Eades, the clinical interview will be conducted during this time. In the case of a one-day evaluation, the parent will be texted roughly thirty minutes prior to the conclusion of testing.
The morning of the evaluation, payment is due in full. Dr. Eades also must have the Consent for Services form prior to working with a client.
Dr. Eades completes all of the testing herself. Behavioral observations provide her integral information that reveal subtleties in functioning, which are invaluable to understanding the individual being tested.

After the Evaluation
Approximately three weeks after the evaluation a feedback session is held with the parents and/or adult client to discuss results and recommendations. This typically is one hour in length. (During the summer months there likely will be a delay in scheduling the feedback session, depending on vacation schedules.) At the feedback, a comprehensive report is provided. In rare occasions, additional information comes to light during the feedback, which then will be included in the report; if this is the case, the comprehensive report will be provided within one week of the feedback. For high school clients, in addition to the parent feedback, a short conference may be held, sharing results in an age appropriate manner. At your request, a meeting with the child’s school can also be arranged at an additional fee.
